Printing individual transmittals:
Non-WisDOT users can print what they need locally.
WisDOT staff can print FDM transmittals locally or click here and
complete the print request form 
to have
transmittals printed in Central Office and sent to you. The Bureau of
Project Development will pay for printing done by the Central Office
print shop.
Department staff should submit completed printing request forms
to: copy-ctr.dbm@dot.state.wi.us
(short version on WisDOT Outlook Express is DOT-DBM Copy-Ctr)
Please add the following information in the form boxes noted
below:
- Box 4 - Order Date
- Box 5 - Due Date
- Box 6 - Total # Originals/Pages (see above)
- Box 7 - FDM Transmittal(s) to be printed
- Box 9 - Order Quantity
- Box 34 - Deliver To
- Box 36 - Contact Name
- Box 37 - Contact Telephone #
